We need your help! If you’re a parent or a provider for young children, take this survey

We need your input! We invite parents and primary care and service providers to take this 15-20 minute survey to help ensure that King County has a developmental screening and referral process that will work for all providers and families.

The survey is open to parents of children under age 25, and providers that serve children and families. This includes medical providers, home visitors, early learning providers, and more. The survey is available online in seven languages and will be open through January 21, 2019. Take the survey now:

Why Developmental Screening and Referral?

Developmental screening and referral is foundational to the well-being of young children, especially those birth through age 5. Cardea is partnering with WithinReach, King County Developmental Disabilities Division, and a coalition of community-serving organizations to learn about developmental screening and connections to services through the voter-approved Best Starts for Kids levy.

Participating in this survey will help inform a universal developmental screening and referral system in King County to ensure all children and their families receive appropriate screening and linkage to supportive services, so more young people grow up to be happy, healthy and thriving adults. Take the survey now!

    • Thank you for your interest! We welcome all responses, but will primarily be using local responses to help inform our strategic planning process for King County.
